Your accountable GP
Every patient registered with us, including children, has an accountable GP. Your accountable GP takes lead responsibility for making sure the care we provide is coordinated and delivered to you.
- We will tell you the name of your accountable GP and how to contact them. If you are a new patient, we will tell you within 21 days of registering.
- If you are under 75, you can tell us which of our GPs you would like as your accountable GP, and we will try to agree. You can also tell us that you do not want one.
- If your accountable GP cannot do this for a significant time, we will tell you and give you a new one if needed.
- If you are 75 or over, your accountable GP also makes sure you get a health check if you ask for one, and works with the other health and social care staff involved in your care.
